This is a great quick reference tool to keep track of your readers. It is designed to use during a one-on-one reading conference with your students. After discussing their books, ask them some of the key questions, record their answers, and then come up with a new strategy of focus together. This form covers all of the basics for non-fiction and fiction texts, allowing for an oral reading sample and strategies to work on section as well.
Pro-Tip: I keep a binder for each reading class I teach with a section designated to each student. All of my students need to have a peer reading conference as well as fill out a conference form before meeting with me to help them focus and comprehend what they read (see some of my other documents for these pages!). This is a great quick reference to bring out during parent meetings and other times throughout the year to track progress!
