This is an informal apraxia assessment. Please note that this is not a standardized assessment; you must use your clinical judgment to decide what your student needs to work on.

Print copies of the data sheet (pg. 3-5) as needed. The stimulus items and pictures cards (pg. 6-21) should be printed in color and laminated for repeated use. I recommend putting the cards on a binder ring for ease of use. Present each stimulus picture (there are 2 per card); the child can independently produce or imitate your model.

The following targets are included:
V in isolation/words, including 4 diphthongs: 16 total
VC: 10 total
CV: 10 total
CVCV: 10 total
CVC: 10 total
CVCVC: 10 total
L-blends: 10 total
S-blends: 10 total
R-blends: 10 total
3 syllable words: 10 total
4 syllable words: 10 total
Intelligibility rating: rate words or sentences, depending on the level of the student
*In the VC, CV, CVCV, CVC sections there are a mix of early and later developing sounds. You may investigate other phonemes/combinations if you wish.
