I use this informal reading record sheet with my reading intervention groups. It helps me stay on track with skills that I am observing, saves time, and serves as a great way to keep my reading records organized. This tool can be utilized with guided reading, small groups, and/or one one one interventions. The form lists several skills to observe before reading, during reading, and after reading. I have also included a running record section to assess accuracy, rate, and prosody. Lastly, there is a section to write the teaching point for that day or week and plans for the next meeting.
