This is a generic document with Microsoft Word comments that will help you modify this structure to support your project or that can show you the options you would have to request something more specific for a negotiable price depending on your needs. If you have familiarity with Word, you will be able to make the modifications you need in response to the prompts I've left in the document :)
It was designed for a first-year teacher, and so the comments and directions in the side-bar are very explicit.
