Knowing how frustrating it is not to have a contact number for a student because they have not turned in their emergency contact sheet, I have created this form for teachers to give out to parents when they are dropping off their child on the first day of school, it’s quick and easy for parents to leave an emergency contact number on the first day of school, while you wait for the emergency form to be returned. There is also a space on the right hand side for the student number. (This is optional only for teachers that assign a number to their student for the school year) Each page has 2 cards, just print as many as you need using your favorite color card-stock paper and cut on dotted line. Have plenty of pen/pencils to give out with the cards so parent won’t have to wait.
