This is a quirky form that doubles as a model for a differentiated instruction strategy. When I walk into a room to observe a teacher, it's nice to get a feel for what they need that day. This form asks "How do you like your toast?". Essentially, I want to know if they want observations only, positive feedback only, simple changes and tricks, or straight to the point concerns. They choose which category they fall under for that day, so I can help them in a way that will benefit them most.
