As a special educator, I feel it is vital to have consistent communication with the instructional staff. I keep a 2 inch binder filled with this 3 hole punched form in a communcation area of my classroom. It helps me to remember to reinforce my staff for what they are doing well as well as give feedback for deficit areas. My staff can in turn ask me anything knowing I have the time to answer after the students leave. As assistants are rarely scheduled during non-instructional time, this tool helps build our relationship and increases effectiveness in the classroom.
