As an ESOL coach, I work with both mainstream and ESOL teachers to help them meet professional goals and improve the quality of their instruction. My style of coaching is very individualized, so that I can meet each teacher's specific areas of need.
I use these forms when meeting with teachers throughout the year. The first form is a goal setting form, allowing me to discuss and record the teacher's specific goals, any obstacles they see in their way, plus supports they will need or want. The form contains guiding questions to aid in the discussion and fields to record the responses.
The second form is for follow-up meetings, to monitor how the teacher feels s/he is moving toward their goals and any successes or challenges they've had thus far. The form contains guiding questions to aid in the discussion and fields to record the responses.
The third form is blank, allowing the coach to use his/her own guiding questions and providing fields to record the answers.
These great forms also allow for both teacher and coach to sign at the bottom (especially great if you have an iPad and stylus) to document the visit.
