This editable document serves as an instrument rental contract between the teacher, student, and parents for the use of a school-owned instrument for a given school year. I have uploaded this as a PowerPoint document so that you can change the school name, add your name, and alter the cost of the rental as needed. All three parties must sign the contract, and I fill out the instrument information at the bottom to keep track of which instruments are in use by which students.
