I've included the pages I have my students glue in to their readers notebook. I use a 5 subject notebook, but you could use a binder or a composition notebook. I've included the word document so you can change the fonts or add and delete as you see fit. These are all completely customizable.
Included you will find:
- Reading Passport (a book log with the requirement of reading from multiple genres throughout the year). This log includes the 10 books that are on the elementary Virginia Reader's Choice for 2014-2015, and the 10 on the primary list (that I assign for extra credit - they have to read all 10 in either first or second quarter in addition to their other required reading).

-Reading Conference boxes - for when you walk around and do individual reading conferences, the students can record your observations and their goals.
-Me As a Reader - I use this as a "Get to know You" as a reader activity. The students write who they were as a reader, who they are, and who they want to be.
-Word Study Options - We use Words Their Way so the students have individual lists. This page gives a selection of choice activities the students may use for their word study.
-How to write a reader's response - this give requirements and a quick checklist of how they will be graded. I use the letter format so the students write me a letter about their reading each week, and I respond back to them.
-Spelling Boxes - as the students spell words incorrectly in their writing, they are asked to write the correct word in the boxes. They are then responsible for spelling the word correctly in their future writing.
-The vocabulary process - I use a combination of Marzano's strategies, and the process described in Teach Like a Champion. I give them 20 words every other week. In a couple weeks I will start adding the vocabulary lists and activities.
