Informal Transition Assessment for students receiving Special Education services, ages 16-22.

A Transition focused assessment that gathers both academic and functional information related to the development of the student’s IEP plan and program.

This Interest Inventory covers:  areas of strengths, areas of need, postsecondary goals, annual goals to make progress towards postsecondary goals, and instruction, activities, and supports necessary to meet those goals.  This Student Interest Inventory is formatted with subheadings to help you complete all parts of the ITP.

2 versions:

• Editable PDF Version - students can type their answers on a computer and print it out
• Lined Print Version - students complete by hand
• Assessment is 4 pages in length (both versions)
