After the Intervention Assistance Team first meets to discuss a student, a period of time follows during which interventions are put into place for the student (usually 4-8 weeks). After the intervention period, the IAT needs to meet again to review the student's progress and determine a new path. This 2-page form leads the team through discussions about the fidelity of the interventions and the data collected during progress monitoring. The form also helps the team make an action plan to follow after the meeting.
