When the Intervention Assistance Team first meets to discuss a student, they need a problem-solving format to guide the meeting. With this 2-page blank form in hand, no additional planning is needed. This form leads the team through the identification of strengths and weaknesses, and it helps the team identify a key area of focus for intervention. The team then develops a goal and brainstorms interventions. The form includes charts for documenting selected interventions and methods for progress monitoring.
