In the Intervention Assistance Team process, observations of the student during class can play a key role in helping to define interventions. This observation form is one of the forms in my series of IAT forms, and it matches the appearance of the rest. It is a 2-page document that has the observer identify target behaviors for observation and secondary concerns. On the front of the form, the observer lists teacher actions and observable behaviors. On the back of the form, the observer describes target behaviors when they are observed. Each part of the form has boxes for observers to fill in specific components.
