I looked all over last year for a good sheet to track my interventions on. I couldn’t find any that I really liked so I ended up keeping all of my interventions in a spiral notebook. I had room to write what we did, what went well, what they were still struggling with, etc. I decided to make my own kit to track my interventions in the upcoming year. I plan to have a binder for each child. There is a sheet for each month where you and the child sit down and make goals for that month together. At the end of the month, there is a box you can check or put a sticker in if they achieved that goal. I plan to let them choose a prize if they achieve their goals for the month. I thought these might also be great to show at parent/teacher conferences. You could also send a copy of the monthly goal sheet home so the parents know what you are working on that month. These would also be great to give to the teacher the following year or to the RTI team. The following items are included in this kit:
