I always start the year with an introduction to the counselor lesson so the kids know who I am and exactly what my job entails (a lot of them have no idea!) I use this in conjunction with my lesson. I talk about how I am a helper and several ways I can help them. This activity is a follow up to see what they learned.
A place for them to put my name (I often write it on the board. I also get a lot of description words in it)
A box for them to say where my office is.
A box for them to put a way or ways I can help.
A box for them to put when I won't keep their secrets (confidentiality).
A large box for them to write something they'd like for me to talk about.
This is a great tool to see how much your students are getting from your lessons as well as help you identify students who may need to see you.
This is an EDITABLE product, so it can be changed to fit your lesson. It includes a younger version with hearts and a version with larger boxes and no hearts for older kiddos.
*BONUS UPDATE* Now includes two additional forms. Includes a simpler form for younger students for just what they want to talk to you about. Space allows for picture drawing. Second is a "what do you need" type form. This is made for older students and is great to collect data on what they want help with!
