This spreadsheet will easily highlight which questions students struggled with. It will also calculate the number of students who answered the question correctly or incorrectly.
I used this with the GoMath curriculum. The chapter assessments tell you common struggles for each question and the CCLS that it corresponds with. You can put that information on the top of the chart in the grey cells. If students got the question correctly, fill the cell with a "y". This cell will automatically turn green. If it is incorrect, it will turn red when you input an "x" into the cell.
You can then use the completed sheets for reteach cycles or data driven lesson planning.
