As an Itinerant Teacher, I have a hefty caseload and it is often difficult to maintain organization, especially when Quarterly Reports are sent home with Report Cards. Therefore, to ease my stress levels, I have added this form that I personalize for each of my students and use it, every session. I am able to keep track of IEP dates, Evaluation dates, Grade Level, Disability information, as well as document the IEP Goals and/or Objectives that I am responsible for updating with student progress. Each progress achieved is listed to the right of the Objective and/or Goal utilized during each session. I am then able to calculate the student’s progress and use this data to update their Quarterly Reports. You can edit this form to meet your liking by adding additional charts for separate IEP Goals and/or Objectives, as well as updating student information to make it accurate to your personalized caseload of students. Parents are thrilled to have data that documents the improvements that their child has made throughout a Quarter or Semester. I never have trouble explaining “how” my student achieved their progress, because I can pull out this form and give administrators, teachers and parents/guardians a visual of their student’s/child’s progress. This form can be tedious, but when it has become habit, it truly enhances the educators’ ability to maintain organization and improves their overall efficiency. Prior to this form, it took me numerous hours to complete progress reports. However, now that each student has their own form, I am able to cut down time spent on quarterly reports significantly.
