Kindergarten Assessment (Sight Words & Reading Street Words):
This data form I use throughout the school year and give to parents for all marking periods. Each student has a booklet with these sight words in them which are reviewed with me and any parent volunteer or room helper throughout the school year.
I mark the letters and words the students do not know with an x, so parents and I know which words they still need to master. This assessment form helps me with report cards; student guided reading groups and parent/teacher conferences.
Classroom Data Sheet Form:
This form I fill out at the beginning of the year after my first assessment and then at each marking period. It gives me a view of how my students are doing overall with their reading sight words and level. I assess if my guided reading groups need to be changed and whether students need to have other services made available to them.
Student Comparison Form:
This form is filled out at the beginning of the year and at each marking period and given to parents with their child’s report card and assessment report. This allows parents and me to see how the student is achieving throughout the school year. It helps us to work out a plan to help students who may be struggling or to give those students who are high achievers more advanced instruction.
Directions for Sight Word Book:
At the beginning of each school year I make a copy of this booklet for each student (cover included). I make each page a different color to make it easier to find a certain page for students and myself.
I use these pages to assess student’s letter & sound recognitions; and Reading Street high frequency words.
I send a copy of the page of words we are working on in student’s homework folders so they can practice with their parents at home. In the beginning of the year I send home the letter & sound recognition, color & number words. I then send the other pages home when we begin to work on those words and units.
During the school year I star the words in student books that they have mastered. I have parent volunteers and/or room helpers work with students on these practice pages. We also get these books out in guided reading and whole group instruction to practice these skills and words.
I transfer student mastery of these skills and sight words on a my data forms for each individual student each marking period . The data forms are sent home with the student’s report card or discussed during parent/teacher conferences.
