This resource is a parent letter to be filled out and sent home after assessment completion. Checkboxes are provided for skills a child needs practice in. This form fosters open communication between school and home, is helpful to inform parents of progress, and provides information to target skills that require reinforcement.
Reading categories include: finger pointing to words when reading, discriminating between letters and words, identifying upper- and lower-case letters, saying letter sounds, reading taught sight words, generating rhymes, retelling a story and tracking print.
Writing categories include: proper formation of letters, writing first and last names, writing name using capital then lower-case, spelling using beginning sounds, ending sounds and vowel sounds, and adding details to writing and illustrations.
Math categories include: writing numbers, knowing number names, knowing counting sequence, counting by 10s to 100, one-to-one matching as counting, greater than/less than/equal to, counting on when adding, addition facts to 5 or 10 and subtraction facts to 5.
