This assessment includes:
numbers 0-10 and 11-20
The first three pages I print out and place in a page protector for the students to have in front of them during the assessment. They could also be placed on card stock and laminated.
Then I use the remaining pages to document the student's mastery of the letters, letter sounds, numbers 0-20, and counting skills by circling the ones they are not able to identify and documenting their progress on their counting skills.
After that I make a copy of the pages and send it home. Now I have data on where the student is at for small group and report card documentation and I have sent home a "Study Guide" for parents to help their child at home and be aware of how their child is progressing in these skills.
Differentiation and Parent Engagement - Check
To assess for knowledge of the sight words, a set of flashcards on index cards can be created for the students to attempt to read as you circle the ones they are not yet able to read fluently. The words can be substituted for the words required by your school or district.
