My district has certain goals as to what levels our Kinder babies should be at before they move on to the first grade. With that in mind, I created this form where I can keep track of all my students with just one page (front and back) for the entire year! This will especially help me with identifying students who need extra help in their basics. Simply copy one for each student and use 3 different colors (I use a highlighter) to keep track of my students progress. There are flash cards as well so you can assess faster and a slider for the syllables to make it less tedious with keeping track of cards.
If students are really struggling I use this form to show parents where they need help and give them copies of the flash cards to help them at home. So far, If students can master this whole form by the MOY students will leave Kindergarten in a level C or higher in reading level, simply because the can identify the very fundamentals. Of course they may need coaching in other reading strategies, but if they have this down 100% it will be easier for them to learn to read.
