This informal assessment measures readiness for kindergarten and may be used repeatedly to record progress and identify areas of need. It measures basic number concepts, print concepts, and pre-reading skills. A two-page document allows the rater to record results easily and the activity cards can be laminated and kept in plastic baggies for repeated use and organization. The only additional item required is a kindergarten level story book.
Directions: Print in black-and-white or color. Cut the cards on the dotted lines. Print the first two pages for each student to be assessed, single- or double-sided. Follow the directions on the record pages to assess student skills and areas of need.
