This is a set of listening center response sheets I use in my kindergarten classroom in our "Listen to Reading" rotation. The children listen to a story, then fill out these sheets.

There are 5 different sheets.
The first one I use at the beginning of the year. There is a place for the title of the book (I usually write that in at the beginning of the year), a place to color a smiley or sad face to say if the reader enjoyed the story, and a section to draw a picture about the story.

The 2nd sheet has a place for the child to draw or write about characters and setting, and then circle if the story was Fiction or Non-fiction.

On the 3rd sheet, the reader writes in the book title, draws/writes about the beginning, middle, and end of the story, then draws/writes about a "text to self connection" they had with the story.

The 4th sheet has a place for Title, Author, and a place to circle what the reader thinks the author's purpose is.

The 5th sheet is a main idea and 3 supporting facts page for non-fiction books.
