This is the perfect tool for common core student accountability. At the beginning of the year, you can have a conference and assess each student through this student/teacher conference. Record actual data and discuss a goal for each student in each of the main areas marked, or that you prefer. (Extra pages included for areas you deem to be most important). Write goal. As student reaches goal, this goal will be marked off by the student.
Each student may be at a different place, or working for a different goal.
At specified times, perhaps grading periods or interims, check for goals.

- I would place them in individual folders where students can get them – as I provide access. STUDENTS HAVE TO BE AWARE OF WHERE THEY ARE AT ALL TIMES according to new common core standards. This is a perfect way for each child to have access to his or her own data and be familiar with DRA levels, how many sight words recognized, and so forth. Also, this tool is perfect for teachers to achieve our new goal of keeping each student accountable - in child friendly terms that students understand and help fill out.
