Kindergarten Speech and Language Screen

Description
This kindergarten screen is designed to identify children who may need further speech and language assessment based on classroom functioning, observation and completing a series of activities. It is intended to be filled out by kindergarten teachers or a staff member in the classroom who is familiar with that child.
Screen looks at:
- social and play skills
- ability to sit during group activities
- grammar and sentence length
- vocabulary skills
- following directions
- comprehension and sequencing
- answering questions
- basic executive functioning
- pronunciation
The activities completed by the child should take 10-15 minutes to complete. The observation portion should not take long, and much of it can be filled in while the children complete the story comprehension portion of the screen (e.g., grammar, sentence length & pronunciation).
Once the form is complete, it is forwarded to the SLP assigned to that classroom. Based on the information provided the SLP will determine if further investigation is needed. This screen has also been used as an observation form by SLPs during classroom observations.
