This is a simple assessment for kindergarten students. It is designed to assess whether they understand line symmetry in relation to shape. It does not deal with color or patterns. I administer this assessment by having students circle the shapes that show symmetry, and putting an "x" through the ones that do not. I use my document camera to project the page, then point to each shape and say, "does this shape show symmetry. If it does, circle it. If it does not, put an "x" through it." My students did very well on this assessment, and it was wonderful to have this little bit of data on their understanding of the concept.
