Here is a quick and easy way to keep students accountable to the lab safety rules you have established in your classroom. If a student violates a lab rule, all you have to do is cut out one of the lab safety tickets, check off the appropriate lines, and staple it to their assignment. This helps to keep track of any students who should not receive full credit on the assignment due to not acting appropriately.
This file has two different versions. The first version has 9 tickets on one sheet, and is completely editable in a word document, and has a PDF file of it as well. The second has 6 tickets on one sheet and includes extra spaces to write in "other," as well as spots for teacher and student initials. It is also editable in a word document, and has a PDF file.
