Description

This document is specifically designed to use when moderate to severe students are rotating through stations that are manned by an adult. The far left side is to code the IEP goal/objective for each activity. The middle section is to indicate the actual activity that will be done. The prompt code allows the adult to indicate the level of prompt that was needed to complete that task. The order box tells the student or adult the order in which they are to transition through each activity. The box beside each station title is to indicate which adult will be at that station. If more than one station activity will be addressed by one adult, that adult's name is indicated beside each. The order number is then the same for both activities. There are more stations on this sheet than are used in a class period, however, this allows for variation when activities change. This self-contained sheet is used to indicate which goals and objectives were addressed each day, the activites used, and at what level of independence the activity was reached. Comments assist me in understanding how the activity went. Hard copy work is attached and the packet is saved and reviewed to record student growth and plan for the next day. This sheet is part of a complete folder that houses the activities and/or worksheets that will be done in each station. The students take their folder with them from station to station. If more than one student is rotating in tandem, they will have the same order schedule with variations in their activities based on their needs. This form works well for my needs.
