This survey allows students to communicate with their teacher their language background and their learning style. Ideally, students would take a learning style or multiple intelligences inventory with this but they could also mark their strengths as they are aware of them. Students also have room to make a language goal for the school year.
Please note that this survey has Spanish and English marked as the primary options for languages because they are the most common home languages in my district. You can easily modify the chart if your students have a different home language.
