This questionnaire is a lifesaver for me when I'm trying to assess and track students' learning skills. I like to have proof to back up my evaluations, and this questionnaire helps students become aware of their own strengths and needs, as well as giving me some physical evidence of students' learning skills.
There are 28 questions with "never, sometimes, usually, or always" checkboxes. Questions are organized by learning skill: independent work, initiative, responsibility, collaboration, organization, and self-regulation. At the end, there are also a few short answer questions for goal-setting.
I use this questionnaire before each report that is sent home to parents, and I keep a copy handy at parent-teacher interviews.
I read through each item, elaborating out loud if necessary, while students check off never, sometimes, usually, or always. Most of my students have been able to complete this very accurately and honestly, and others check "always" for every statement, indicating to me that they may not be aware of their own strengths and needs as a learner, which is something I discuss with children and their parents.
