This checklist has been recently updated using the Ontario Ministry of Education guidelines from the Growing Success Document.
Even though this document was generated from the Ontario Curriculum documents, it would work in any educational format.
There are two parts to this document: a daily learning skills checklist and a weekly tracking sheet.
The daily learning skills checklist covers the following six learning skills: responsibility, organization, independent work, collaboration, initiative and self-regulation. Students will mark their own progress at the end of class, checking 'N' for needs improvement, 'S' for satisfactory, 'G' for good and 'E' for excellent. There is a spot for teacher initials and comments.
The weekly summary guide allows the students to summarize his or her behaviour for the week. It also allows the student to record various assessment opportunities throughout the week and track his or her goals. There is a spot for teacher initials and comments.
This is a great way to encourage students to take responsibility for their own learning and to communicate their progress using learning portfolios.
