I have spent two years looking over other templates, pulling key features, and adjusting them in order to save time, be practical, and efficient. Read any book on education or teaching and it will stress the importance of objectives! With this in mind, the first section is a form that I find useful to create content and language objectives. Simply circle your skill/Depth of Knowledge, state what students will be learning, how they will learn that skill and how you will measure the learning. Use those key items to build your objective, and write it in the space provided at the top of the page.
The next section provides a snap shot of your lesson. Check off what learning styles you will be featuring, how you plan for students to retain the days learning, how you are grouping students, and how you will assess them.
The back page of this template is where you can get into more detail and the sequencing of your lesson. I like to write an estimate of time for each section, so that I can keep my self on track.
