This assessment was created in conjunction with our Let’s Try… Calming through Curiosity box. We wanted students to name their emotion and document it both before and after using the box. You can use this assessment with our product or any calming kits/items you have students using for regulation.
The student fills in the front of the card and educators fill in key information on the back of the self-assessment (we recommend printing back-to-back). Educators should be documenting date, time with the box, and notes about what was happening prior to the student becoming escalated. The assessment cards are kept and used at intervention meetings, team meetings, conferences, etc. The data can be used to track frequency of escalations, potential triggers, and trends.
