This is a quick and easy assessment to see which, letters and sounds a child knows and does not know. You can do this assessment a couple of different ways. The first way is to show the checklist to the child and have the child say the letter/sound as you point them. The other way is for you (teacher) to keep the checklist in front of you and place the letter list in front of the child and have them say each letter/sound as they point to it. At the top there is a place to put the date, this way you know when you gave it. At the bottom there is a place to keep the score and there is a place for the person giving the assessment to put their initiates (so you can keep up with who gave it).

Page 4- is in black and white and the letters are in order.
Page 5- is in black and white and the letters are mixed up.
Page 6- letter list (lowercase and are in order)
Page 7- letter list (lowercase and are mixed up but, it follows the checklist order)
Page 8- letter list (uppercase letters and are in order)
Page 9- letter list (uppercase letters and are mixed up but, it follows the checklist order)
