We are told to be proactive rather than reactive, but many teachers have trouble remembering what proactive steps to take at a moments notice.
In an attempt to remedy both teacher frustration and office referrals, I created this level 1 behavior response tracking sheet. This sheet can be added to the referral to “prove” that many interventions took place prior to sending the student to the office or they could be used to help track interventions in the classroom.
Just write the students name and check off the interventions you used to help deter unwanted behavior. Add the problem behavior to the bottom of the checklist.
