I made these for my personal classroom. They are intended for students to use when checking out library books from the library. Instead of having students take their A.R. folder to the library, the students can take their card and it can also be their pass to the library. You can print slides 3 and 4 front and back. Slide 3 is to list their A.R. or lexile level so each student can easily remember what level to checkout from the library. I conference with my students each week. If a child needs to move up or down in their reading level, I mark out the level prior and list the new level below in the correct 8 week column. Slide 5 and 6 are for the teacher’s notebook. This serves as a master copy if a child loses their card or if it becomes misplaced. You can modify this document based on your school’s requirements for A.R.
