Purpose: I originally designed these flashcards for a 4th grade student who did not know his home address or phone number. I was concerned, what would happen if this child got lost, and could not tell someone his home address?
Objective: Help students learn their personal information such as their home address, phone number, and birthdate.
How to assemble: Print flashcards on both sides of the page, flipped on the short edge. I recommend printing them on cardstock.
How to use: In the beginning, it will help for an adult to work alongside the student to learn the information. Start with a few cards at a time, and gradually build. Once the student becomes comfortable with the information, he or she can practice quizzing themselves each day for 10 – 15 minutes. They will create a pile of correct responses & incorrect responses to track their progress. They can also have another student quiz them.
Developmental level: These cards can be used for students of any age who need to learn their personal information. For students with memory or processing issues, repetition is key. Make sure student practice every day, either at home or at school. Once the student has learned this information, more cards can be added, such as: days of week, or months of the year.
