I print this on two sides of paper, so there are eight slots. I then place them onto clipboards and allow line monitors and restroom monitors to first warn students that are misbehaving, allow the student to fix their behavior, then if they violate a hallway/restroom rule again, the monitor marks their name and the abbreviation for what they did. There are only 4 abbreviations/violations to make it as simple for small children as possible. It is still appropriate for older students too, as I had fifth grade students as line monitors too when I taught that age level. Great for hallway and restroom management and students love the responsibility.
