This is a cut and paste worksheet that allows students to match standard form and slope-intercept form by converting. Notes are provided on the top of the benefits of each form. Students then can choose the preferred form and graph each line. Their is additional room in each box for student to identify the slope, and x and y intercepts if the teacher directs students to do so. This sheet is 8 problems, front and back followed by a double set of equations to cut out. The assignment is 4 problems in standard form students will use to calculate the x and y intercepts and use the intercepts to graph each line, then convert each equation to slope-intercept form and check using their graph.
