This Linear Form Conversion with Graph and Summary set of sheets was created for an Algebra 1 or Algebra 2 class where students are practicing converting from one form to another. Each of these sheets gives a graph, and students are responsible for finding point-slope form, slope-intercept form, and standard form along with x-intercept, y-intercept, and slope for each equation represented. Students are then asked to write a short summary about how to find each of the forms. A quick answer sheet of the slope-intercept and standard forms is included.
