This activity includes:
8 different Linear Functions. Each represented as an equation, a graph, a table, and two points. You can cut out each card and randomly hand out to students and have them find the equation of a line from the piece of information they are given. Then have them find the three other students that also have their equation. As a group they can fill out the graphic organizer.
Another option is to have them fill out the graphic organizer that includes the directions for the students to create step-by-step instructions on how to write an equation of a line given a table, then a graph and finally from two points. The students can make a poster, foldable or study guide.

You can also use this product as a centers activity and students can match together the correct table, graph, equation and points on the line.
