This activity is a great way to introduce your students to writing and thinking about equations in slope-intercept form that apply to real-world situations. Each of the four pages walks the students through identifying the independent and dependent variables, completing a T-chart for the described situation, identifying the rate of change and the starting amount, writing and graphing the equation, and finally answering questions based upon the model.

These are simple situations which allow students to use prior knowledge to think about when slopes and y-intercepts are positive or negative. This can be used with higher-level students as an introduction without much prior knowledge of slope-intercept or as an assessment after instruction for struggling learners.
