I am a title I reading tutor and have created these forms as a tool to benefit the flow of my "Daily 5-esque" small group system. The first is a form I use to track where my students have been throughout the week and ensure they have conferenced with me twice, read to someone five times, and visited every other station at least once.
These will not fit everyone's needs, but I thought I would post it for anyone who may benefit. Instead of the typical Daily 5 center "Listen to Reading", we do "Read and Record" so we can hear what we sound like as readers and focus on how to improve our fluency. We also do not do "Work on Writing" since my curriculum is strictly reading intervention (although writing does makes an appearance quite often in "Word Work").
