I have my literature circle groups use this evaluation sheet at the end of each meeting period. This helps hold students accountable even if you are not sitting in with the group the whole time.
I laminated the page and have each student keep it in their Literature Circle folder along with an expo marker, the book their group is reading, and a handout explaining each Lit. Circle role (this is another product in my store). By laminating a class set, each page can be reused over and over rather than printing a new one for each student every time their group meets. I tell the students they are not allowed to wipe the form clean, I will do that after I take a look at them.
This system has worked very well in my 4th and 5th grade classrooms, allowing me to sit in with one group per day while still ensuring the other groups are doing their job.
