This is an easy sticky note reading response form which can be used for literature circles or independent reading to track student understanding of the story. Teachers can provide specific prompts and students write on sticky notes and stick them in the box next to the chapter that they are working on. Students may stick multiple sticky notes atop one another. When students fill these out as they read, the information that they have written down is helpful when they write a story summary or reviewing important concepts in the story for tests. I have even allowed students to use their notes for quizzes! Another great way to use this is for sustained silent reading and home reading as they help to keep the students on track with their reading and I can ask them more probing questions based on their sticky note responses.
