Use this handout during a lesson/unit on moon phases. I've attached this as a word doc so you can add dates to each box if you'd like, or leave it blank for students to fill in.
One page includes boxes for almost a full month of moon observations - students have to shade the moon phase they see in the sky, record the time, date, and type of phase.
On the back of the journal there is series of questions that focus on how the moon changes over the course of a month, as well as questions on eclipses, a day/year on the moon, and what a new moon is.
Since it's a word doc, you can make the changes necessary to work best for your teaching needs.
