You can use this in any class, but especially Learning Strategies, OLC, English etc.
I give this out right after they get their midterm report cards - so it's all fresh in their heads. It's good to check-in at the halfway point and see how they're doing.
It asks them to look at their classes OVERALL - every course - so it provides really thorough feedback for them.
The handout goes through:
- It looks at their current mark and their goal marks - but more importantly, what are they going to do to achieve that
- It asks them for my course specifically - what are the obstacles / plan / prediction
- a self-monitoring checklist for things they're doing (adding handouts to binders etc.)
- to make them smile, at the end they have to fill out a "certificate of achievement" for themselves for ANYTHING :)

It's a great metacognitive activity for them.
