This is a versatile form that can be used with ANY story!
The form allows the students to independently reflect on the book that they have just read.
The students are instructed to identify the genre, characters, setting, and main idea of the story. They are also encouraged to find five new words from the story and to define them. They must also reflect on whether they enjoyed the story or not, if they'd recommend it to a friend, and if there are any parts that they would change.
This is a great form to have handy at a reading center or somewhere that the students can easily grab one throughout the day and evaluate each book they have read throughout the year.
