Hard as it may be to admit, even with excellent classroom management systems in place, there are students whose behaviors disrupt the classroom and keep themselves (and others) from achieving their academic potential. This packet contains 9 forms that redirect and document the day-to-day disruptors that test and break the classroom rules. The zip folder includes both .pdf and .docx formats so the purchaser can make adaptations if needed.
There are 4 redirection activities in which the student either copies an essay about choice making or looks up the definitions of certain positive behavior words. These are designed to allow the student time to reflect on his actions, and reinforce the idea that he has control over his behavior and is making choices with consequences.
Also included in the packet are 5 other forms to help teachers document incidents and show trends of behaviors.
4 - Redirection Activities for use with disruptive students
2 - Reflection Forms for incidents involving multiple students, and for students who refuse to complete an assignment
3 - Documentation Forms to track student behavioral choices and show trends to parents
This packet is designed for use in upper elementary and middle school grade levels.
